A Senior Care Manager coordinates family, community, or financial resources to help the older person navigate between the levels of the continuum of care to arrive at the best, most independent situation for them. Senior Independence Bridge® provides senior care managers who can:

  1. conduct an assessment with you and your family to identify the need for services and design an individualized Care Plan to meet those needs
  2. arrange and monitor reliable, reasonably priced in-home personal care or home maintenance services
  3. review financial, legal, or medical issues and offer referrals to seniors' specialists who may help avoid future problems and conserve assets
  4. communicate with family members, physicians, and others as requested
  5. consult on appropriate living arrangements with special consideration given to needs and restrictions
  • help a family choose the best facility after the crisis of a hospitalization
  • assist with moving a loved one from one level of care to another
  • help with choosing the best facility to meet the changing needs for long term lifestyle
  • assist with relocation including housing, integration into the community resources, and moving details
  • be available 24 hours a day
